If you’ve just started playing Purgatorio, you’ll quickly realise that surviving isn’t just about having good weapons or fast reflexes. The items you carry can completely change the outcome of a run. Using the right healing item at the perfect moment or saving a combat buff for a difficult enemy often makes the difference between escaping and starting over.

The game currently features 10 different usable items, and while some are incredibly powerful, others shine only in specific situations. Knowing when to use each one is just as important as knowing what it does.

This guide explains every item in Purgatorio, what each one does, and which items are worth prioritising during your runs.

This is easily one of the strongest survival items currently available.

Rather than slowly recovering your health, the K-Corp Ampoule immediately heals you back to full, making it perfect for emergencies or difficult encounters.

When should you use it?

Don’t waste it after taking minor damage.

Instead, save it for moments when:

  • You’re close to dying.
  • A boss fight is about to begin.
  • You’re surrounded by dangerous enemies.
  • You’re deep into a run with no other healing available.

Since it provides a complete heal, timing is everything.

Diluted Ampoule

Effect: Healing amount depends on your current Ampoule Toxin Stacks.

Unlike the standard K-Corp Ampoule, this version isn’t always equally effective.

Its healing changes depending on how many toxin stacks your character currently has, meaning you’ll get different results throughout a run.

Because of this, it’s a little harder to use efficiently.

Best use

Treat it as a situational healing item rather than your primary source of recovery.

Bandage

Effect: Grants double healing for 10 seconds.

At first glance, Bandages might not seem particularly exciting.

However, they’re actually very strong when combined with other healing sources.

Instead of healing immediately, Bandage temporarily boosts the amount of healing you receive during the next 10 seconds.

Best use

Use a Bandage before consuming another healing item whenever possible.

This lets you get much more value from your recovery.

Moon Chunk

Effect: Restores 90 Sanity.

Sanity is just as important as Health in Purgatorio.

If your Sanity drops too low, surviving becomes much more difficult.

Moon Chunk is currently one of the best recovery items for keeping your Sanity high throughout longer runs.

Best use

Use it whenever your Sanity starts reaching dangerous levels rather than waiting until it’s completely empty.

Maintaining your Sanity is much easier than trying to recover from a critical situation.

Stimulant

Effect: Grants 2 Damage Up stacks for 10 seconds.

If you’re looking to deal as much damage as possible, this is one of the strongest offensive items in the game.

For a short duration, your attacks become significantly more powerful.

Best use

Save Stimulants for:

  • Boss fights
  • Elite enemies
  • Large enemy groups
  • Time-sensitive encounters

Using it during normal fights often wastes its short duration.

Throwing Knife

Effect: Applies Bleed to enemies.

Rather than dealing massive damage instantly, Throwing Knives slowly wear enemies down through Bleed damage over time.

They’re especially useful against enemies with large health pools.

Best use

Throw the knife early in a fight so Bleed has plenty of time to deal damage while you’re attacking normally.

Stick

Effect:

  • Restores 5 SP
  • Grants 1 Poise

Stick is a small utility item that offers modest recovery alongside a useful Poise bonus.

It won’t completely change a run, but it can be surprisingly useful when you need just a little extra support.

Best use

Use it whenever you’re running low on SP or need an extra Poise stack before engaging tougher enemies.


Canned Coffee

Effect: Restores 5 SP.

Simple, reliable and easy to understand.

Canned Coffee exists purely to replenish your SP.

While it isn’t flashy, maintaining your SP means you’ll have access to your abilities more consistently throughout a run.

Best use

Drink Coffee before long fights or whenever your SP starts getting low.

Rations

Effect:

  • Restores 10 Health
  • Restores 10 Stagger

Rations provide a balanced recovery instead of focusing entirely on one stat.

Although the healing isn’t huge, restoring both Health and Stagger makes this item useful in many different situations.

Best use

Perfect for recovering between fights without spending one of your stronger healing items.

Ammo Bundle

Effect: Restores 25 Ammo instantly.

If your build relies on firearms, Ammo Bundles become incredibly valuable.

Running out of ammunition during a difficult encounter can quickly end an otherwise great run.

Best use

Don’t use it the moment you’re missing a few bullets.

Instead, wait until your ammo supply is running dangerously low to maximise its value.

Best Items in Purgatorio

While every item has its purpose, a few clearly stand above the rest.

K-Corp Ampoule

The strongest healing item in the game.

A full health restoration can completely turn around difficult situations and save otherwise failed runs.

Moon Chunk

Sanity management becomes increasingly important during longer runs, making Moon Chunk one of the safest items to keep in your inventory.

Stimulant

Extra damage is always valuable, especially during boss encounters where ending fights quickly often means taking less damage overall.

Purgatorio’s item system rewards smart decision-making more than quick reactions. Every item has a purpose, but knowing when to use it is what separates successful runs from failed ones.

If you’re looking for the safest items to prioritise, it’s hard to go wrong with K-Corp Ampoule for survivability, Moon Chunk for Sanity recovery, and Stimulant for boosting your damage during difficult encounters. The remaining items are more situational, but they can still become lifesavers when used at the right moment.
