The Stalker is one of the creepiest encounters you’ll come across in Animal Hospital. Unlike other threats that actively chase or attack you, the Stalker plays mind games instead. It quietly watches from around corners, disappears almost instantly, and punishes players who aren’t paying attention.

The first time you see it, there’s a good chance you’ll instinctively look directly at it—and that’s exactly what you shouldn’t do.

If you’re wondering how the Stalker works and how to avoid losing Sanity, here’s everything you need to know.

The Stalker is a tall, shadowy black rat-like creature that randomly appears throughout the hospital.

Rather than attacking you head-on, it hides around corners and waits for players to notice it.

As soon as you look directly at the Stalker, it quickly disappears behind the wall.

Even though the encounter only lasts a second or two, looking at it comes with a penalty.

What Happens If You Look at the Stalker?

Looking directly at the Stalker causes you to lose:

  • 10 Sanity

After being spotted, it immediately retreats and disappears from view.

If you run over to where it was standing, you won’t find it anymore—it vanishes almost instantly after being seen.

Because Sanity is an important resource, repeatedly triggering the Stalker can become a real problem during longer runs.

How to Know the Stalker Is Nearby

Fortunately, the game gives you a couple of warning signs before you accidentally look at it.

When the Stalker appears, you may notice:

  • The lights in the nearby room flicker.
  • A small sound effect plays.
  • The creature briefly peeks around a corner before hiding again.

If you notice flickering lights or hear an unusual sound, it’s worth slowing down before turning the next corner.

Stalker Spawn Locations

The Stalker can appear in several different parts of the hospital.

Known locations include:

  • The Lobby
  • Between the Emergency Wing Entrance and the Office Store
  • In front of the Medical Store
  • In front of the Emergency Wing Store
  • Inside the Surgery Room
  • Behind the Vending Machine
  • Behind a Potted Plant

Since its appearances are random, there’s no guaranteed location each time you play.

How to Avoid the Stalker

The good news is that the Stalker doesn’t have to damage your Sanity.

The trick is avoiding direct eye contact.

Instead of sprinting around every corner, move a little more carefully whenever you’re exploring the hospital.

If you suspect the Stalker is nearby:

  • Slow down before turning corners.
  • Keep your camera angled slightly downward.
  • Watch the edges of your screen rather than looking straight ahead.

This gives you a chance to notice the Stalker without staring directly at it.

Don’t Look Directly at It

This is the most important rule.

If the Stalker appears, resist the urge to center your camera on it.

Instead, keep it near the edge or corner of your screen while continuing to move.

Think of it like catching something in your peripheral vision rather than making eye contact.

Many players lose Sanity simply because their first reaction is to turn and see what made the noise.

Watch for Its Legs

One helpful trick is to watch the lower part of corners rather than looking directly into them.

The Stalker’s legs are often enough to tell you it’s there.

Spotting just a small part of the creature lets you react before accidentally looking straight at it.

It’s a subtle strategy, but it becomes much easier with practice.
