Story Progression Achievements
These achievements unlock naturally as you move through the main story:
- Again – Complete the Prologue.
- Clever – Solve the Train Puzzle.
- Survivor – Defeat your first enemy.
- Tunnel Vision – Complete Act 1.
- Watcher – Complete Act 2.
- All Aboard – Complete Act 3.
- Scum – Survive the encounter with your past.
- Commute – Complete Act 4.
- One Eye Open – Complete Act 5.
- Forgive Forget – Finish the game (any ending).
Simply play through the story while being mindful of puzzles and survival encounters, and you’ll unlock all of these naturally.
Endings Guide
There are two endings in Post Trauma, and your actions during the final sequence will determine which one you get.
How the Endings Work:
Shortly before the final boss, you’ll face a choice involving Jill (the spider creature).
You can manually save in the area with the pin puzzle to quickly unlock both endings without replaying the whole game.
Ending A – It’s Over
What to do:
Kill Jill by driving fast and hitting her with the subway before reaching the final abomination.
Result:
- The abomination survives, and you face the darker Ending A.
Ending B – It’s Done
What to do:
- Drive slowly (or not at all), and do NOT kill Jill.
- She will later appear during the final cutscene and destroy the abomination for you.
Result:
- The abomination is defeated, leading to the slightly more hopeful Ending B.
Bonus Ending Achievement:
Tail End – Destroy the abomination (part of Ending B).
Save right before the encounter, get Ending A, reload your save, and adjust your actions for Ending B to save time.
Collectibles Guide
There are two main collectible types you’ll need for full completion:
- Weapons – Find all five weapons.
- Used Tapes – Find all four tapes.
Both must be collected in a single playthrough to unlock:
- Defender – Found All Weapons.
- Remember – Found All Tapes.

Act 1 – Subway
Used Tape A
Found immediately after leaving the subway tracks, look for a room off the main path.
Crowbar (Weapon)
Unmissable.
Found in the first save room during the prologue/tutorial sequence.
Act 2 – Hospital
Sledgehammer (Weapon)
Unmissable.
Found inside a locked toolbox in the Emergency Consultation Room.
Used Tape B
Located in Room 51, the room with the hanging blood bag.
Pistol (Weapon)
Found in Room 46, tucked away inside.
Act 3 – School
Shotgun (Weapon)
Solve the Clock Puzzle inside the Headmaster’s Office.
It’s a tricky one — pay close attention to clock hands and room hints.

Located on a table in the Study Hall, near the Jupiter miniature planet.
Broken Spear (Weapon)
Found in the Church area after using the Smelling Salts.
Act 4 – Ruins

Located at the start of the maze section.
Stick to the right-hand wall to find it fairly quickly.
