This is by far the most complex The Coffin of Andy and Leyley Ending to get. It requires making multiple specific choices, including a critical one all the way back in Episode 2.
In my opinion, this is also the most disturbing ending in the game. I’ll keep spoilers to an absolute minimum, but if you’ve played even a little, you probably already have an idea of what you’re walking into.
The Coffin of Andy and Leyley Ending Guide (v3.0.2)
Step 1 – Hitman
During Episode 2, when you’re at the park with the hitman and Ashley, you’ll get two options:

“shoot”
-
“SHOOT SHOOT SHOOT”
This is the single most important choice in the entire game.
You MUST pick “shoot”, which makes you only fire once at the robber.
If you go with “SHOOT SHOOT SHOOT”, you’ll lock yourself out of this ending (and a few others).
Step 2 – Olive Branch
Still in Episode 2, when you’re in the basement, Ashley has to decide:

“Trust the Trinket”
-
“♥♥♥♥ That”
You MUST choose “Trust the Trinket” so that Andrew stays behind to guard the parents.
During this, Andrew will talk to his mother, who will give him an Olive Branch. He will then have two choices:

“Reflect upon it”
-
“Refuse”
You MUST pick “Reflect upon it”.
This decision locks you into the Decay Route. If you refuse, you’ll be on a completely different path.
Step 3 – Bang Bang
Ashley eventually falls asleep and has a vision.
At the end of this nightmare, you get two choices:

“BANG BANG MF”
-
“Nah”
Pick “BANG BANG MF” to stay on track for this ending.
Step 4 – Gas Station
I haven’t confirmed if this is 100% necessary, but it worked for me, so I’d do it just in case.
In Episode 3, when you bump into the cops at the gas station, you’ll get two excuses:

“Stomach Issues”
-
“First Date Nerves”
Pick “First Date Nerves” for some… interesting life advice.
Step 5 – Wedding Ring
This is easy to miss, so PAY ATTENTION.
Right after the gas station, you’ll be in a forest section where you have to push rocks.
Look for an area with:

A top path
-
A bottom-right path with footprints
SAVE YOUR GAME before moving any rocks, just in case you mess up.
Now, go to the footprint path FIRST. If you do this right, you’ll find a box with a wedding ring inside. Grab it.
Step 6 – I am Andrew, Not Andy
Shortly after Step 5, Ashley will ask who you are.
Your options:

“Andrew”
-
“Andy”
YOU MUST PICK “ANDREW”.
Andy is dead. You’re on the Decay Route now, stick with it.
Trust me, the alternative is much worse.
Step 7 – Hold Onto It
You made it.
At this point, you’ll stub your toe and see something familiar—the wedding ring.
Your choices:

“Chuck it”
-
“Hold onto it”
Pick “Hold onto it”.
And that’s it. You’re locked in.
If you followed every step, congratulations! You just unlocked one of the most messed-up endings in the game.
